AN independent inquiry has been ordered into how Lucy Letby was able to murder seven babies and attempt to kill six others. It is to be hoped that this will not bring yet more anguish to the bereaved families.

When staff, clinicians and consultants expressed concern, nobody listened so nothing happened. One of the several consultants who raised concerns about Lucy Letby was ordered to apologise to her in writing.

Too often in the public sector, the NHS and government the repetitive statement emerges “lessons have been learned” but decisive disciplinary action seldom happens. The guilty parties are usually retired on enhanced pensions with a large severance packet instead of being kicked out the door.

The independent inquiry must name those guilty of gross negligence and ensure they leave in disgrace with not a penny more than they are entitled to.
